Payment for all services is due at the time services are rendered.
We work with several insurance carriers, and we are happy to file claims for you; however, if we do not work with your insurance carrier, we can only accept out-of-network assignments. Please remember that the information your insurance carrier sends us is only an estimate of your benefits. You are responsible for any unpaid balance after your benefits have been applied. You must provide a 10% nonrefundable down payment to schedule surgery. If you need to cancel, please do so at least five business days before the scheduled surgery date. Failure to do so will result in a fee of 10% of the total surgery fee.
